Step 3 Complete Intake Forms

Once your appointment is scheduled, you’ll receive a secure email from Osmind:

  • Digital intake forms
  • HIPAA consent documents
  • Screening questionnaires (if needed)

Please complete all forms at least 24 hours before your appointment

Step 4 Attend Your First Appointment

Telehealth Appointments: A secure video link will be sent 24 hours before your session

During your first visit, we will:

  • Discuss your current symptoms and goals
  • Review your history and any previous treatments
  • Build a personalized treatment plan tailored to your needs
